Hospital to Home service


Hospital to Home program is now delivered by the Housing Hub

The Hospital to Home service was established to support the discharge of people with disability from hospital to housing that aligns with their housing needs and preferences.

As part of our continuous efforts to enhance and streamline our services, the Hospital to Home program is now delivered by the Housing Hub

The Housing Hub was established by the Summer Foundation in 2017 to help connect people with disability to the accessible housing market. Since then the Housing Hub has grown to offer specialist support services for people with disability.

The team delivering the Hospital to Home service remains the same, the eligibility criteria remains unchanged and this service continues to be available for free.
