Discharge Follow-up Service
This service will make phone contact with people with disability with complex needs who are being discharged quickly from hospitals to make sure their supports and essential services are in place.

Housing Brokerage Service
This project will support hospitals we are already working with to find housing for people with disability who are stuck in hospital with no discharge destination.

Phone Outreach Project
This service will connect with people with disability to check on their health and wellbeing, and provide practical information and support with problem solving.

Personal Protective Equipment Initiative
This initiative has been established in direct response to a Summer Foundation survey of NDIS participants which found that many are having difficulty getting Personal Protective Equipment (PPE).
An information page about PPE has been set up and 2 online stores where NDIS participants can buy reasonably priced PPE are now open.

Leaving Hospital Well Community of Practice
A national platform for health professionals to share their expertise, identify best practice for working with younger people with disability and complex support needs and provide a supportive learning environment within the evolving context of health and the NDIS.
