The Practice Guide is a tool for anyone working with people 65 years and younger, living in or at risk of living in nursing homes. It provides guidance for people supporting and advocating for young people in nursing homes, particularly when it comes to them accessing the National Disability Insurance Scheme (NDIS).
The NDIS is an important reform. But connecting young people living in nursing homes to the NDIS is time intensive work and can also be very challenging for all involved. We address many of these challenges by drawing on the knowledge, wisdom and experience from the Summer Foundation’s ‘NDIS Connections Project’.
There are many reasons why someone may end up living in a residential aged care facility. But with the right support, young people living in nursing homes can have greater options to live more independently.
You can download a two page summary of the Practice Guide.
This includes an explanation of what the NDIS offers young people in aged care and what aged care providers can do to help get young people into the NDIS.
