1. Get an NDIS plan or change your plan

A nursing home manager can help you get an NDIS plan. If you have an NDIS plan, it may need changes. Your support coordinator can help you do this. You can ask for a face-to-face meeting with an NDIA planner. They will write down your needs and your goals.

 


2. Work out your housing and support needs

You need to work out where to live. You need to work out who you want to live with. Your support coordinator or an allied health worker (e.g. an OT) can help you. They can help you to plan your move.

 


3. Work out if the NDIS will pay for your housing

Your support coordinator will help you look for different types of housing. They will help you work out if NDIS will pay for the housing you need. They will tell the NDIA about your housing needs.
