NDIS Connections Project

The Summer Foundation’s NDIS Connections Project was established to assist young people in residential aged care facilities to connect with the NDIS.

The NDIS is a significant reform rolling out across Australia for people with disability to receive the funding they need to live in
 the community. The Summer Foundation established the NDIS Connections Project in February 2014 to find young people in nursing homes in the Barwon (Victoria) and Hunter (NSW) NDIS trial sites, later expanding into the ACT and Perth Hills trial sites.

Our research shows that many younger Australians under the age of 65 years in aged care facilities are not aware that they are eligible for the NDIS. Our aim is to make contact with people to explain the NDIS, and to provide assistance to connect with the Scheme. We also want to make sure the National Disability Insurance Agency (NDIA) is aware of what it takes to connect all young people to the NDIS.

Connecting young people living in nursing homes is time-intensive, and there can be challenges for all involved. Through our NDIS Connections Project, we aim to work collaboratively with residential aged care facilities, the NDIA and people with disability and their families, to ensure that the process of accessing the NDIS is as easy and as quick as possible.

Our Practice Guide details how to support young people in nursing homes to connect to the NDIS. The following diagram from the Guide shows the steps that a person may need to take to access the full benefits of the Scheme.

The NDIS will provide personalised support and funding to eligible participants. But participants may also need support to connect, prepare, plan and then implement their plan. As most young people living in aged care facilities will be eligible for the Scheme, our goal is to ensure they can access the full benefits of the Scheme to increase their independence and improve their quality of life.
