Working with people in hospital

  • December 10th, 2019 9:30 am
  • Sunshine Hospital, 176 Furlong Road, St Albans VIC

The Government has set the targets that by 2022 no people under the age of 65 will be admitted into aged care.

To meet this target, support coordinators and allied health professionals will assist people with complex needs in hospital to get and implement an NDIS plan that can support discharge back into the community.

Summer Foundation are facilitating this workshop to build the capability of support coordinators and allied health professionals to work effectively with people in the hospital setting. This will help ensure that people can discharge with the appropriate health, NDIS and mainstream and informal supports in place.

Workshop participants will have an understanding of the key drivers of hospitals and the NDIS, and how to work collaboratively across the interface.

This workshop will be presented by Penny Paul, Senior Policy and Research Officer at the Summer Foundation. Penny, an expert in SDA eligibility and author of the Summer Foundation’s NDIS Housing resources, also leads UpSkill – an initiative to build the capacity of the NDIS workforce.
