The Summer Foundation wants to explore the role of Support Coordinators in assisting people with disability to successfully exit hospital and return to the community with appropriate supports.
We are seeking Support Coordinators who have worked with a person with disability during a hospital/rehabilitation admission and supported them during the discharge planning process, to participate in a workshop.
The insights from this workshop will inform resources and training to help develop a skilled support coordination workforce equipped to support people with disability through the hospital discharge process to help them avoid admission to aged care.
All Support Coordinators that participate will be offered a free training session from the Summer Foundation UpSkill program.
