Building your skills to work with people in hospital (Mildura)

The Government has set a new target which will see no people under the age of 65 being admitted into aged care by 2022. To meet this target, NDIS support coordinators and allied health professionals will assist people in hospital with complex needs, to get and implement an NDIS plan that can support discharge back into the community.

Why should you attend?

The Summer Foundation is facilitating this workshop to build the capability of support coordinators and allied health professionals to work effectively with people in the hospital setting. This will help ensure that people can discharge with the appropriate health, NDIS and mainstream and informal supports in place.

Outcomes

Workshop participants will understand:

  • The key drivers of hospitals and the NDIS, and how to work collaboratively across the health and NDIS interface

This workshop will be facilitated by Penny Paul, the Summer Foundation’s Senior Policy and Research Officer and SDA eligibility expert.
