The Ian Potter Foundation We gratefully acknowledge the support of the Ian Potter Foundation who have recently agreed to fund a Pilot Information and Carer Support Network for Families of Younger People in Residential Aged Care. This project will enable us to locate, connect, inform and support the families of people under 60 years of age with acquired disabilities currently living in aged care in Victoria.
WCF Thomas Charitable Trust We gratefully acknowledge the assistance of the WCF Thomas Charitable Trust which is managed by ANZ Trustees in supporting the From A Home to Their Home project and for seed funding provided during our inception that enabled us to work towards locating, connecting and informing people under 60 years of age with acquired disabilities currently living in aged care, and their significant others.
Grey Grey have been instrumental in helping us reach out and communicate with families, workers and policy makers to drive our mission of fostering the development of the next generation of services and solutions that promote the health and well-being of people with acquired and late onset disabilities and empower them to participate in the community.
