"From a Home to their homes" DVD and Information Booklet |

The Summer Foundation has developed a DVD and information booklet, outlining a variety of accommodation and support models that documents the experience of six people. The booklet provides information to consumers and families about various community housing models for younger people with differing degrees of care needs. A description of each of the accommodation and support models is included with the story of a young person who has moved from an aged care facility into the community.
