Quality of Life evaluation


The Summer Foundation has been engaged by the Department of Human Services to undertake a Quality of Life evaluation for individuals receiving support services through the my future my choice program.

The evaluation will assess whether there has been an improvement in the quality of life for young people who have received my future my choice  supports, and it will also look at factors that foster or impede successful outcomes in order to inform future service delivery. The researchers will interview individuals and their families who may fall under the following categories:

  1. People who have moved or are going to move from aged care to my future my choice community based housing with supports

  2. People who have received individualised support packages whiles remaining in residential aged care accommodation settings

  3. People who have been diverted from entering aged care to reside instead in community based housing or at home with supports.


Research will involve analysing some of the information obtained through the my future my choice planning process, as well as an interview with the participant and/or a family member/carer in the next 12 months.


The Department of Human Services has now sent a letter to all my future my choice participants inviting them to participate in the evaluation.


Your help is needed
We are seeking your help to ensure that the evaluation is comprehensive by obtaining as many perspectives as possible from both people with disabilities and their families who have participated in the my future my choice process.


Many people in this target group are unable to open letters and respond to invitations independently. If you are in contact with a my future my choice participant it would be very much appreciated if you could support them to make an informed decision and either accept or decline the invitation to participate in the evaluation.
